Students who have the approved accommodation of receiving books in alternative formats should complete this form. This form needs to be submitted at least 2 weeks prior to classes starting. You must also submit a copy of the receipt for the hard copy of the book as a proof of purchase. We will only begin processing your requests for books when this form has been completed in its entirety and receipts have been submitted. 

*Please note, many textbook publishers require students to purchase their accessible e-book version in lieu of releasing a separate PDF/ accessible copy. If your textbook is available in an accessible e-book version, you should purchase this version first.
